Second Trimester Care — Weeks 14 to 27

The second trimester is often the most comfortable phase of pregnancy. Critical events: detailed anomaly scan at 18–22 weeks, gestational diabetes screening at 24–28 weeks, fetal movement perception, and growth establishment. Surveillance is calibrated to your risk profile.

What Happens in the Second Trimester

Energy returns as first-trimester nausea recedes. The uterus rises above the pelvic brim. Fetal movements (quickening) felt 18–20 weeks (first pregnancy) or earlier (subsequent). Fetal growth accelerates. Cardiovascular, renal, and metabolic systems adapt further.

The Anomaly Scan (18–22 weeks)

The most detailed routine fetal anatomic assessment in pregnancy. Performed per ISUOG standards using AI 3D/4D sonography by Dr. Shreya Patel. Identifies major structural anomalies, soft markers, placental position, cervical length, and fetal growth. Approximately 50–70% of major anomalies detected at this scan.

Gestational Diabetes Screening (24–28 weeks)

75g OGTT — fasting, 1-hour, and 2-hour values. Earlier screening (16–20 weeks) for high-risk women: BMI ≥25, family history, prior GDM, prior macrosomia, PCOS. Diagnosis triggers nutritional intervention, monitoring, and pharmacologic management if needed. Tight control significantly improves outcomes.

Fetal Echocardiography

Detailed cardiac scan at 20–24 weeks for indications: diabetic pregnancy, family history of CHD, abnormal findings on routine scan, IVF pregnancy in some cases, advanced maternal age, certain teratogen exposures. Performed in our fetal medicine programme.

Common Symptoms and Concerns

Backache, leg cramps, mild oedema, varicose veins, heartburn, constipation — most are normal and managed with lifestyle measures. Concerning symptoms: vaginal bleeding, severe headache, visual changes, severe abdominal pain, premature contractions, watery discharge, decreased movements after 24 weeks.

Frequently Asked Questions

When will I feel the baby move?

Typically 18–20 weeks in first pregnancy, earlier (16–18 weeks) in subsequent pregnancies. Fluttering or bubble-like sensations initially, progressing to clearer kicks.

Can I sleep on my back in the second trimester?

Generally safe in early second trimester. After 28 weeks, left lateral position is preferred for optimal venous return. Use pillows to support side-lying.

What is the anomaly scan checking for?

Major structural anomalies of brain, heart, spine, abdominal wall, kidneys, limbs; soft markers for chromosomal conditions; placental position; cervical length; fetal growth; amniotic fluid.

What if the anomaly scan finds something?

Most findings are minor or normal variants. Significant findings are discussed thoroughly with you, additional imaging may be arranged, and referral to specialists is provided as needed. Decisions are always yours, with full information and support.

Should I take a glucose tolerance test even without symptoms?

Yes. Universal screening is recommended because most gestational diabetes is asymptomatic and only identified by testing. Early detection allows intervention before fetal effects develop.
