Pregnancy by Trimester β€” First, Second and Third

Pregnancy unfolds in three trimesters, each with its own clinical questions, monitoring priorities, and supportive needs. First-trimester care confirms viability, screens for early complications, and establishes the baseline pregnancy profile. Second-trimester care includes the anomaly scan and routine screening tests. Third-trimester care monitors growth, prepares for delivery, and addresses late complications when they arise. The transition between trimesters is not a hard line β€” but the care priorities meaningfully shift.

In short: Pregnancy is three trimesters, each with its own checkpoints: dating and early screening (0–13 weeks), the anomaly-scan window (14–27), and growth with delivery planning (28–40). Visits and scans follow that logic.

Our approach to trimester-by-trimester care

Every pregnancy at Balaji Horizon is monitored on a structured schedule calibrated to risk profile β€” RCOG schedule for low-risk pregnancies, with additional checkpoints added for any identified risk factors. We discuss what to expect at each visit before the visit, so patients know what is happening and why.

First-trimester care (weeks 1–12)

Dating scan, viability confirmation, baseline bloods, screening discussion (NT scan + combined screening or NIPT), nutritional and lifestyle counselling. Early pregnancy is also when many anxieties peak β€” we provide accessible answers between visits.

Second-trimester care (weeks 13–27)

Anomaly scan (typically 18–22 weeks), gestational diabetes screening, fetal movement counselling, continued blood pressure and weight monitoring. The mid-pregnancy phase is when many patients begin to feel more confident β€” we maintain monitoring without manufacturing anxiety.

Third-trimester care (weeks 28–40)

Growth scans, blood pressure surveillance for preeclampsia, group B strep discussion, delivery planning conversations, birth preferences, breastfeeding preparation. The frequency of visits increases through the third trimester.

Each trimester at a glance

StageMain focus of care
First trimester (weeks 1–13)Confirming and dating the pregnancy, early screening, folic acid, and managing early symptoms.
Second trimester (weeks 14–27)The detailed anomaly (TIFFA) scan, glucose screening, and the start of fetal movements.
Third trimester (weeks 28–40)Growth and position checks, birth planning, and watching for warning signs.

Frequently asked questions

How many antenatal visits and scans will I have across the three trimesters?
A typical low-risk pregnancy involves roughly 8–10 visits and a few planned scans β€” a dating/viability scan early, the NT scan around 11–13 weeks, the detailed anomaly (TIFFA) scan at 18–22 weeks, and growth scans later. The schedule is individualised to your history.
Which trimester is the most important?
Each matters for different reasons β€” the first for dating, viability and early screening; the second for the anatomy scan and glucose testing; the third for growth, position and birth planning. Structured care means nothing is missed at the stage it matters.
When does extra (high-risk) monitoring usually begin?
It depends on the reason. Some risks are known from the start, such as a previous caesarean, diabetes or twins; others emerge from screening or a later scan. Where added monitoring is needed we explain why β€” see high-risk pregnancy care.
Which symptoms should prompt me to call rather than wait for my next visit?
At any stage, contact us promptly for heavy bleeding, severe or persistent abdominal pain, a severe headache with visual disturbance, high fever, leaking fluid, or β€” later in pregnancy β€” reduced fetal movements. These need assessment rather than reassurance at home.
Planning your pregnancy? See our antenatal care schedule β€” a structured, visit-by-visit ANC programme aligned with FIGO and ISUOG guidance.
