For Endometriosis Consultation
Symptom diary (pain timing/severity, bleeding pattern), prior ultrasound/MRI reports, prior operation notes, current medications, AMH report if recent, clear fertility plan.
For IVF / Fertility
Menstrual diary, prior ultrasound and AMH/AFC reports, semen analysis (male partner), prior IVF cycle records, infectious screening, recent thyroid and prolactin reports.
For Pregnancy / Antenatal
Prior pregnancy records, current obstetric records if continuing care, recent test results, ultrasound reports, medication list, vaccination history.
For Any Consultation
