Gynaecological Diagnostics — Advanced Ultrasound and More

Gynaecological diagnostics begin with the consultation — but advance imaging answers questions a consultation alone cannot. Modern transvaginal ultrasound, performed to ISUOG protocols by experienced operators, characterises most pelvic pathology. MRI adds where ultrasound is inconclusive or surgical planning requires more detail. The order matters: skipping the careful history and examination in favour of immediate complex imaging is rarely the right move. The diagnostic plan is structured, sequential, and built around the specific clinical question.

Our approach to gynaecological diagnostics

Diagnostic decisions follow from the consultation, not the other way around. The clinical question shapes the imaging modality; the imaging informs the management plan. We use expert ultrasound as the primary diagnostic modality for most pelvic concerns — with MRI, CT, or hysteroscopy added selectively where ultrasound alone is insufficient.

Advanced gynaecological ultrasound

The Balaji Horizon ultrasound service performs structured examinations to ISUOG protocols. For endometriosis (IDEA protocol), ovarian masses (IOTA classification), uterine pathology, and pelvic floor concerns, the specific sub-protocols are applied. Reports are structured — every required parameter is documented, not just summary impressions.

When to add further imaging

MRI is added for deep infiltrating endometriosis, complex adnexal masses, surgical planning in specific cases, and where ultrasound findings are inconclusive. Hysteroscopy is added for uterine cavity assessment where ultrasound suggests pathology or where infertility/recurrent loss requires direct visualisation.

Diagnostic topics

Guidelines we follow

  • ISUOG practice guidelines on gynaecological ultrasound
  • IOTA group classifications for adnexal masses
  • ESHRE/ESGE consensus on diagnostic hysteroscopy

Where this fits

Diagnostics feed general gynaecology care, endometriosis diagnosis, and fertility evaluation.
