What Is the 3D/4D Wellbeing Scan?

The 3D/4D wellbeing scan uses GE Voluson’s advanced volume rendering technology to produce detailed three-dimensional images of the fetus in real time — 4D means live, moving 3D. It is not purely a bonding or cosmetic experience; it provides clinically meaningful information about fetal wellbeing in the second and third trimester.

When to Book — Why 26–32 Weeks Is Optimal

Timing is critical for quality HD Live 3D/4D imaging. The optimal window balances subcutaneous fat development (needed for surface rendering) against amniotic fluid volume and fetal position.

30–32 weeksBest detail for the bonding scan — face complete, subcutaneous features vivid. Still excellent amniotic fluid volume for imaging.
> 34 weeksFetal head descends, fluid reduces, limbs compress against each other. Imaging quality diminishes. Not recommended as the primary 3D/4D appointment.

Why Not Before 26 Weeks?

Before 26 weeks, the fetus has not yet deposited subcutaneous fat beneath the skin. In HD Live mode the face appears hollow and skeletal. The imaging does not reflect true facial features. For optimal surface rendering, waiting until at least 26 weeks is always recommended.

What the 3D/4D Wellbeing Scan Does Not Replace

This is an important clinical distinction that parents should understand before booking. The 3D/4D wellbeing scan is a complementary study — it adds valuable information but does not substitute for structural or risk-based assessments.

This scan does NOT replace:

  • The 18–22 week anomaly scan (TIFFA) — detailed structural survey
  • Growth biometry with EFW and serial centile tracking
  • Doppler studies when clinically indicated
  • NT scan and first trimester chromosomal screening

This scan uniquely provides:

  • Formal fetal wellbeing scoring (BPP) in the third trimester
  • Behavioural assessment — breathing, tone, movement patterns
  • 3D surface rendering of fetal face and palate
  • AFI documentation and placental assessment
  • Bonding experience with HD Live photorealistic imaging

Is the 3D/4D scan just for bonding, or does it have clinical value?

Both. At Balaji Horizon the 3D/4D scan includes a formal fetal wellbeing assessment — biophysical profile scoring, AFI measurement, placental assessment, and fetal behavioural observation. In addition to the photorealistic face images, the scan generates clinically useful data. It can also detect facial clefts, positional deformities, and fetal behavioural abnormalities — but it remains complementary to, not a substitute for, the structural anomaly scan.

Why does my baby’s face look unusual on the scan?

Fetal hands, placenta, umbilical cord loops, or the uterine wall may partially obscure the face. Fetal position also affects image quality significantly. Most unusual appearances are positional rather than structural. The operator will attempt gentle manoeuvres to reposition the baby for a clearer view. Returning after 30 minutes or at a later date may also improve imaging if conditions are unfavourable.

Is 3D/4D ultrasound safe for my baby?

Yes. 3D/4D imaging uses exactly the same ultrasound wave principles as standard 2D scanning. GE Voluson systems operate within WFUMB and ISUOG safety guidelines. There is no ionising radiation involved. Decades of safety data and systematic reviews confirm no adverse effects on the fetus from diagnostic ultrasound at standard clinical output levels.

Will I get a photo of my baby?

Yes. HD Live 3D/4D still images are provided to parents. For the wellbeing scan, a short video clip of fetal movement may also be offered, subject to image quality on the day. The clinical wellbeing report with BPP score, AFI, and fetal assessment is also provided at the same appointment.

Can this scan detect Down syndrome or chromosomal conditions?

No. Chromosomal screening is performed at 11–13+6 weeks via the NT scan and Combined First Trimester Screening. The 3D/4D wellbeing scan at 26–32 weeks is not a chromosomal screening test and cannot diagnose genetic conditions. If you have concerns about chromosomal conditions, please discuss NIPT or amniocentesis options with your specialist.
