What Is a Dating & Viability Scan?

A dating and viability scan is the first formal ultrasound of pregnancy, typically performed between 6–10 weeks of gestation. It is not purely a reassurance scan — it provides critical clinical information that shapes all subsequent obstetric care.

What the scan assesses:

  • Intrauterine location of pregnancy — rules out ectopic implantation
  • Fetal heartbeat (cardiac activity confirmed from approximately 6 weeks)
  • Crown-rump length (CRL) — the most accurate method for gestational age dating in early pregnancy
  • Gestational sac size and morphology — sac shape and growth pattern
  • Number of embryos — single, twin, or multiple pregnancy; chorionicity assessment in twins
  • Yolk sac — presence and morphology
  • Uterus and ovaries — fibroid mapping, ovarian cysts, adnexal findings
Why accurate dating matters: All subsequent screening tests — NT scan, TIFFA anomaly scan, growth assessment, and planned delivery — depend on accurate gestational dating established here. A 5-day dating error at 8 weeks can become clinically significant by 20 weeks.

When to Have This Scan and What to Expect

Gestational AgePrimary PurposeKey Findings
6–7 weeksHeartbeat confirmation and sac identificationFetal pole visible; cardiac flicker from ~6 weeks
7–8 weeksCRL measurement + embryo morphologyMost accurate CRL for early dating; limb buds visible
8–10 weeksRefined dating + twin chorionicity if applicableFull CRL, dichorionic vs monochorionic twin assessment

Preparation:

🔍

Scan Route

Transvaginal scan preferred at 6–8 weeks for superior resolution. Transabdominal from approximately 8 weeks with a full bladder.

🕐

Duration

15–20 minutes. Report provided immediately, including estimated due date (EDD) calculation.

🍴

Fasting

Not required. For transabdominal scan, a comfortably full bladder improves image quality at 8–10 weeks.

📋

Bring

Last menstrual period (LMP) date, any previous scan reports or pregnancy records.

Frequently Asked Questions

  • At what week is the dating scan most accurate?
The dating scan is most accurate between 7–10 weeks of gestation. A scan at 6 weeks confirms viability and heartbeat, but CRL measurement is most precise at 7–8 weeks. Dating before 6 weeks or after 13 weeks is less accurate.
  • Can a dating scan detect ectopic pregnancy?
Yes. An early pregnancy scan raises strong suspicion for ectopic pregnancy if no intrauterine gestational sac is seen in a symptomatic patient with a positive pregnancy test. The combination of an empty uterus and adnexal findings requires urgent clinical management.
  • Is a transvaginal scan safe in early pregnancy?
Yes. Transvaginal ultrasound is safe throughout pregnancy and provides significantly better image resolution than transabdominal scanning at 6–8 weeks. There is no evidence that diagnostic ultrasound causes harm to the embryo or fetus at any gestational age.
  • What if no heartbeat is seen at 6 weeks?
The absence of cardiac activity at 6 weeks does not confirm miscarriage. The heartbeat may not yet be detectable, especially on transabdominal scanning. A repeat scan at 7–8 weeks is required before any clinical conclusion about pregnancy viability is made. Premature diagnosis of miscarriage before 7–8 weeks is a recognised clinical risk.
  • Does this scan confirm my due date?
Yes. The estimated due date (EDD) calculated from CRL at 7–10 weeks is more accurate than LMP-based dating and is the date used for all subsequent clinical planning — including the NT scan, TIFFA anomaly scan, growth scans, and delivery planning. ISUOG recommends CRL-based dating when a reliable first trimester scan is available.
